United States Visa Application for Filipino Travelers
Filipino citizens planning to visit the United States are required to obtain a U.S. Visa before traveling. The most common type for short visits is the B1/B2 Visitor Visa, which allows entry for purposes such as tourism, visiting family or friends, business meetings, or medical treatment. Pre-Visa Application Guidelines
- Attention: Due to high volume of applications, visa processing may take several weeks. Apply early.
- Applications must be submitted by the applicant or an authorized representative with proper authorization letter.
- Ensure all documents are complete before visiting any office for submission.
- Missing or incomplete requirements may result in rejection or delays.
- Do not finalize flight or hotel bookings until visa approval.

- Copy of Travel Insurance covered the whole travel period
- ALL PHOTOCOPIES SHOULD BE PRINTED IN A4 SIZED PAPER
- IF HAVE PRIOR TRAVEL PLEASE INFORM US AND SUBMIT A COPY OF YOUR ETICKETS

(if have any)2 pcs 2x2 inches colored with white background (aside from the photo uploaded online on the DS 160 form)
DS 160 form (visa application form) and applicant information sheet -Travel agency will be the one to online and print the application form
Bank Certificate Original - Personal Account
Bank Statement (Within the last 3 months) - Personal Account
Original PSA Birth Certificate
Original PSA Marriage Certificate(if married)

- Original Certificate of Employment (stating position, tenure, monthly and annual salary)
- Copy of Company ID
- Copy of Latest Income Tax Return (BIR 2316)
- Copy of Mayor’s/Business Permit
- Copy of DTI / SEC registration – all pages with the names of incorporators
- Copy of Latest Company Income Tax Return – (BIR 1701 / BIR 1702-RT)
- Copy of BIR Certificate of registration – (BIR 2303)
- Copy of Latest Company Income Tax Return – (BIR 1701/A)
- Copy of Valid PRC ID/IBP ID
- Original School Certificate
- Copy of Student ID
- Copy of Retirement certificate
- Copy of Senior ID
- Original Affidavit of support – of the guarantor
- Documents of the guarantor – please refer to the above requirements
- Proof of relationship – PSA Birth Certificate, Marriage Certificates
Additional Documents: Optional and only if you have
1. Proof of assets - land titles, vehicle registrations, shares of stocks and other financial documents
2. Credit card statements (within the past 3 months)
NOTE:
1. All documents must be ORIGINAL and with photocopy
